EARLY REMOVAL OF JADELE AND PPIUCD, SOCIO DEMOGRAPHIC DETERMNANTS AND SIDE EFFECTS
Abstract
Objective: To find out the prevalence of early removal of Jadelle and Postpartum Intrauterine Contraceptive Device (PPIUCD) and the socio-demographic factors and complications involved in early removal of these IUDs in a tertiary care hospital.
Method: A cross sectional study was carried on in the Department of Obstetrics and Gynecology, Combined Military Hospital Malir, Karachi Institute of Medical Sciences, Karachi, from 1st January 2026 to 31st March 2026. Women who presented for removal of Jadelle implants or for PPIUCD during the study period were consecutively sampled. Questionnaire was used to gather information on age, parity, educational level, occupation, place of residence, socioeconomic status, duration of contraceptive use, reasons for removal, and side effects experienced by the respondent. Early removal was defined as stopping before the recommended duration of removal. Data analysis was done with SPSS software version 26. The Chi-square test was used to determine the association between the socio-demographic variables and early removal, and p<0.05 was regarded as significant.
Results: A significant number of women asked for early Jadelle and PPIUCD removal. Younger age, low education level, living in rural areas and low socioeconomic status were most common socio-demographic risks for early discontinuation. Irregular menstrual periods, headache, weight gain or loss and arm discomfort were among the most commonly reported side effects of Jadelle users, while excessive vaginal bleeding, lower abdominal pain, dysmenorrhea, and vaginal discharge were among the most commonly reported side effects of PPIUCD users. Family pressure, the wish for another pregnancy and poor counselling was also a factor in early removal.
Conclusion: A number of socio-demographic factors and side effects of treatment affect early removal of Jadelle and PPIUCD. Routine counselling, proper follow-up and management of side effects can increase continuation rates and therefore effectiveness of LARCs.
